We have provided a new patient form for you to print and fill out at home at your leisure. Simply call us at (423) 928-8359, set up an appointment that is convenient for you, bring in your prepared new dental patient form and we will care for you in a time valued manner.

Your Initial visit

When you visit our office for the first time, we want you to feel the difference. Our team has taken extra care to design a comfortable and relaxed environment. From our warm and welcoming rooms to our state-of-the-art technology, our goal is to make you feel comfortable while making your smile beautiful!

Here’s some information of what you can expect during your initial visit at Sophisticated Smiles. We will perform an examination of your teeth and gums, discuss pertinent medical and dental histories, take necessary x-rays (our computerized x-rays reduce radiation up to 90%) and/or do a teeth cleaning.

Please arrive 20 minutes early to have enough time to fill out your health history form or print out our online new patient form at home and bring that in with you. We also request that you also bring a list of medications you are taking. When you make an appointment the time that we set aside for you is your time only. We see our patients individually and value your time and dislike keeping you waiting. We ask that your also respect our time. If you unable to keep your appointment please notify us at least 48 hours in advance.

Emergencies

Occasionally dental emergencies do occur. When they do, contact the office immediately. We will always make time available to treat your emergency as soon as possible.

All of us at Sophisticated Smiles look forward to seeing your smile! When you visit our dental office you will experience an ambiance that is relaxing and comfortable while you enjoy music from our headphones and seat in one of our massage chairs.
